Written Consent for Disclosure

A completed consent form from a child allows Chemeketa Community College to provide information regarding enrollment and academic progress information to the parent paying for college.  Chemeketa Community College supports the intent of the Department of Justice Division of Child Support and will make every effort to provide information in a timely and efficient manner.  To provide a clear understanding to parent and child, Chemeketa Community College has guidelines that will clarify and answer many common questions and concerns.

Always be prepared to show photo ID when requesting information in-person.

Written Consent for Disclosure Form

  1. The child must submit a Written Consent for Disclosure Form to Chemeketa Community College Enrollment Services Office on the Salem campus (Building 2, Room 200) in-person with photo ID
    • The parent may NOT submit the form for the child
    • Chemeketa does not accept these forms by mail or fax
  2. The Written Consent for Disclosure Forms are valid for one year from the date signed
    • The child may rescind consent at any time via My Chemeketa
  3. The Written Consent for Disclosure Form automatically expires upon the child’s 21st birthday
  4. Chemeketa does not collect attendance and cannot provide this information
  5. Chemeketa does not report grades until the end of the term
    • Grades are not mailed, but are available on My Chemeketa, as long as the student is in good financial status with the college
    • Chemeketa does not release grades over the telephone. 
  6. The paying parent may request a copy of the student’s final grades (at the end of term) or verification of enrollment (during a term) at Enrollment Services with photo ID
    • Please fill out this form completely before submitting your request
    • Pick up requests will be processed within one business day

The final grades and schedule of classes will be mailed to the paying parent’s address provided on the Written Consent for Disclosure Form

If the paying parents’ address changes during the term, the child must submit a new Written Consent for Disclosure Form with the new address in person with photo ID to Enrollment Services.  

The Child Attending School Confirmation Form

  1. The child must submit the Child Attending School Confirmation Form to Chemeketa Community College Enrollment Services on the Salem Campus (Building 2, Room 200) in-person with photo ID
    • The paying parent may NOT submit the forms for the child
    • Chemeketa does not accept these forms by mail or fax
    • If the student attends another campus or center they may submit their documents to that campus or center.  The documents must be routed to Salem Campus for processing and  this may create a in delay processing.
  2. The Child Attending School Confirmation Form will be completed while you are at Enrollment Services
  3. Chemeketa does not report grades until the end of the term
  4. Make sure that the Written Consent for Disclosure is turned in either with this form or prior to submitting the confirmation form.
    • Failure to have a Written Consent for Disclosure on file prior to our office processing your enrollment confirmation may result in the Division of Child Support rejecting your documentation.
    • We cannot accept another form of records release in lieu of the Written Consent for Disclosure.
  5. Full-time enrollment is defined at Chemeketa at 12 or more credit hours each term; Half time enrollment is defined as 6-11 credit hours
  6. Satisfactory academic progress at Chemeketa is defined as maintaining a cumulative grade point average of 2.0    
  7. New students in their first term of enrollment are considered in good academic standing
  8. We cannot verify enrollment or academic progress in non-credit programs (i.e. GED programs or high school programs)
