How to Apply

For the 2012-13 school year, applicants must submit the following documents
1. Chemeketa Admission Application
Apply Online
2. Chemeketa CAMP Application
Download Application. Application deadline is March 16, 2012.
3. Personal Statement
Written in English, 1-2 pages, double-spaced, typed.
4. CAMP Recommendation Forms
Two recommendation forms completed by people (not family members) who know you, your work, and your goals. You might ask a teacher, a counselor or an employer.
5. High school or GED Transcript
If not completed, you will be required to submit a copy of an unofficial transcript. Upon graduation, an official transcript must be submitted.
6. Placement Test Scores
All applicants are required to take this test as a part of their CAMP application.
7. Complete Income Verification and Release Form
Included in the application
8. Proof of Eligibility
Migrant or seasonal farm worker: Present proof of qualified farmwork such as W-2 forms or check stubs.
or
Migrant Education Program (MEP): If you participate(d) in this K-12 school program, submit a current certificate of eligibility (COE).
or
National Farm worker Jobs Program (NFJP) of Section 167 of WIA: Provide valid proof of participation.
9. Copy of Student AID Report (SAR) for 2012-2013. Free Application for Federal Student Aid (FAFSA).
