How to Apply
| For the 2012-13 school year, applicants must submit the following documents | |

| 1. | Chemeketa Admission Application Apply Online |
| 2. | Chemeketa CAMP Application Download Application. Application deadline is March 16, 2012. |
| 3. | Personal Statement Written in English, 1-2 pages, double-spaced, typed. |
| 4. | CAMP Recommendation Forms Two recommendation forms completed by people (not family members) who know you, your work, and your goals. You might ask a teacher, a counselor or an employer. |
| 5. | High school or GED Transcript If not completed, you will be required to submit a copy of an unofficial transcript. Upon graduation, an official transcript must be submitted. |
| 6. | Placement Test Scores All applicants are required to take this test as a part of their CAMP application. |
| 7. | Complete Income Verification and Release Form Included in the application |
| 8. | Proof of Eligibility Migrant or seasonal farm worker: Present proof of qualified farmwork such as W-2 forms or check stubs. or Migrant Education Program (MEP): If you participate(d) in this K-12 school program, submit a current certificate of eligibility (COE). or National Farm worker Jobs Program (NFJP) of Section 167 of WIA: Provide valid proof of participation. |
| 9. | Copy of Student AID Report (SAR) for 2012-2013. Free Application for Federal Student Aid (FAFSA). |
