Dental Assisting Program
Fall 2007 Application Packet
In compliance with the Family Educational Rights and Privacy Act, Chemeketa College releases only very limited information regarding students. All Dental Assisting students, including those who have filed a Request for Non-Disclosure of Student Information Form, should be aware that some confidential information may be shared. Contracts with practicum placement sites require that the following information be made available about students placed at these sites: CPR certification, immunization status, TB screening test results, and criminal background check. This information is needed for purposes of student and patient safety.
Before the first day of fall term, all students admitted to the Dental Assisting Program will be required to pass a criminal background check which fulfills the requirements of clinical sites. If a student does not pass the criminal background check, program registration will not be possible. If a student is arrested during the time he/she is enrolled in the Program, he/she must notify the Dental Assisting Director of the arrest. The student's status in the Program will be reviewed by the Director, and a possible outcome of the review may be the student's inability to continue in the Program.
Every effort is made to limit access to confidential student information to those who have a need to know. For more information regarding Non-Disclosure of Student Information, contact the Registrar office at 503.399.2511.
The Dental Assisting Program is in compliance with the guidelines and regulations set forth by the Centers for Disease Control and Prevention (CDC), the Occupational Health and Safety Administration (OSHA), and the Oregon Health Division. These agencies have established written rules and procedures to protect students and program staff from ionizing radiation, bloodborne and other infectious diseases, and hazardous materials. Additionally, students and program staff must be immunized and/or tested for measles, mumps, rubella, tuberculosis, Hepatitis B, and chicken pox prior to contact with patients in an effort to minimize the risk to patients and dental personnel. Declination forms are available upon request. Failure to provide information requested to carry out the above policies may result in an applicant's being denied admission to or a student's dismissal from the Program.
This program is accredited by the Commission on Dental Accreditation and has been granted the accreditation status of ĝapproval without reporting requirements. The Commission on Dental Accreditation will review complaints that relate to a program's compliance with the accreditation standards. The Commission is interested in the sustained quality and continued improvement of dental and dental-related education programs but does not intervene on behalf of individuals or act as a court of appeal for individuals in matters of admission, appointment, promotion, or dismissal of faculty, staff, or students. A copy of the appropriate accreditation standards and/or the Commission's policy and procedure for submission of complaints may be obtained by contacting the Commission at 211 East Chicago Avenue, Chicago, IL 60611-2678 or by calling 1.800.621.8099 extension 4653.
Chemeketa Community College is our community's resource for quality education in a changing world, delivering opportunities for adult literacy, opening the door to all levels of college, and creating centers of excellence in technical training, workforce development, and business support. We are a college community enriched by the diversity of our students and staff. Each individual and group has the potential to contribute in our learning environment. Each has dignity. To diminish the dignity of one is to diminish the dignity of us all.
