MedicationsParents/guardians must provide written instructions signed by the parent/guardian and physician (or proper prescription label on medicine container giving the physician's name) on a special form located in your child's classroom. Information requested is: name of child, name of medication, time to be given, dosage, reason for medication, possible side effects, and termination date for administering medication. All medication will be kept in the locked medicine box located in the kitchen. Teachers will record on the medicine form each time medication is taken. Aspirin and over-the-counter medication may be taken by the child, if they provide the medication and parents/guardians write instructions for administering on the medication form. If your child has allergies (bee stings, food, plants, etc.) medic alert, asthma, or medical information of which the center should be aware, please include this information on the emergency file form and discuss this with your child's teacher during the preschool visitation. |
