Confidentiality
Disability Services considers student information confidential and only releases records and information in compliance with laws governing the release of such information.
Use, Retention, and Re-release of Records
Documentation from a qualified professional about a student’s disability is secured in the Disability Services office and does not become part of your academic record.
All disability related information is kept in separate files for each student and housed in secure file cabinets.
Only authorized personnel working at Disability Services have access to these files.
When a student is no longer enrolled at Chemeketa, Disability Services maintains your records for three (3) years. After three years, Disability Services student records will be destroyed.
Disability Services does not re-release records unless required to do so by law.
Disability Services may release records generated by our office, only when a student has signed an "Authorization for Release and Disclosure of Information" form.
A student may request to review the contents of his/her own file. This must be done with a staff person from Disability Services present. All information in the file is the property of Disability Services.
