Documentation of Disability
Learning Disabilities
Learning disability documentation must include an assessment that meets the following criteria:
- Qualified professional evaluation that includes intellectual assessment and achievement test scores
- Documentation must be within the past three years for students coming out of high school (if tested using adult assessment tools after the age of 18, reports may be accepted within five years of the assessment)
- Examples of acceptable assessments:

- Woodcock Johnson Psycho-Educational Battery—Revised, Standard AND Supplemental Batteries (WJPEB-R)
- Wechsler Adult Intelligence Scale—Revised (WAIS-R) or 3rd Edition (WAIS III)
- Stanford-Binet Intelligence Scale ( 4th Edition)
- Wechsler Intelligence Scale for Children—Revised (WISC-R)
- Achievement Tests:
- Woodcock Johnson Psycho-Educational Battery— Revised, standard AND Supplemental Batteries (WJPEB-R)
- Wechsler Individual Achievement Tests (WIAT)
- Stanford Test of Academic Skills (STAS)
- Scholastic Abilities Test for Adults (SATA)
To receive accommodation for a learning disability the assessment must indicate the following:
- Full Scale/broad cognitive scores on the Intellectual Assessment must fall in the average range or higher
- A significant difference (-1.5 standard deviations or more) must exist between the full scale/broad cognitive score and the standard score in one or more of the achievement areas

Attention Deficit/Hyperactivity Disorder

Deafness/Hard of Hearing
- Audiological assessment
- Physician’s statement that includes diagnosis, nature of disability, severity, and functional limitations

Low Vision/Blindness
- Visual assessment and diagnosis Physician’s statement that includes diagnosis, nature of disability, severity, and functional limitations

Traumatic Brain Injury

Psychological Disorders
- Mental Health Professional’s statement that includes diagnosis, nature of disability, severity, and functional limitations (including DSM IV diagnosis)
- If the condition fluctuates, recent documentation may be required
Physical Disability
- Physician’s statement that includes diagnosis, nature of disability, severity, and functional limitations
- If the conditions fluctuates, recent documentation may be required
Communication Disability
- Licensed speech pathologist’s statement that includes the diagnosis, nature of disability, severity, and functional limitations
- If the condition fluctuates, recent documentation may be required
Who needs documentation of a disability?
Any student who needs accommodations because of a condition that places substantial limitations on his/her functioning
in an academic setting.
What is documentation?
Documentation is information (current within the past three years) about the disability provided by a doctor or professional
trained in the specific area of disability.
When must this documentation be provided?
Documentation must be provided to the Disability Services office two weeks before the student can receive services.
Where is this documentation kept?
Documentation is kept in a separate confidential file within the Disability Services office and is not part of the student’s academic record.*
Why does CCC need documentation of disability?
- To determine eligibility and effective accommodations for each student
- To meet funding sources requirements
- To follow the national guidelines of the Association of Higher Education and Disability (AHEAD)
*This office follows the Health Insurance Portability and Accountability Act (HIPAA) and Family Educational Rights and Privacy Act (FERPA) Standards.
