Information regarding services
The purpose of Disability Services is to assist individuals who experience disability with accessing campus programs and services. Disability Services meets this goal by providing academic adjustments and auxiliary aids as afforded by law under the Americans with Disabilities Act of 1990 (ADA), Section 504 of the Rehabilitation Act of 1973, and in accordance with college policies and procedures.
When Requesting Accommodations, Your Responsibilities Include:
- Providing current documentation from a qualified professional that identifies the nature of your disability, how it substantially limits a major life activity, and outlines the functional limitations related to that condition.
- Paying for any medical/psychological assessments and/or record request fees.
- Requesting your accommodations from your accommodation specialist each term.
- Following the accommodation procedures as described in the Disability Services Student Handbook.
- Communicating with each of your instructors regarding your accommodation needs and providing them with a current Disability Services “Approved Academic Accommodation Letter” each term.
- Following the student Code of Conduct and maintaining the Standards of Satisfactory Academic Progress as outlined in the Chemeketa Community College schedule.
- Resolving accommodation/services disagreements as outlined in the Disability Services Student Handbook.
- Contacting Disability Services if you cannot make any scheduled Disability Services appointment (i.e., Accommodation meeting, testing, and in-class services).
Disability Services Responsibilities and Services Include:
- Determining eligibility for accommodations based on disability documentation and functional limitations.
- Providing appropriate and reasonable academic accommodations and support services.
- Providing referrals to, and collaborating with, college faculty, staff, and programs.
- Providing referrals to off-campus services and resources.
- Collaborating with other professionals who are working with you regarding your disability (i.e., your doctor, therapist, and/or vocational rehabilitation counselor).
- Maintaining confidential Disability Services records while you attend Chemeketa Community College and for a period of three years thereafter.
