When do I disclose my disability?

It depends on how it affects your ability to do the job. For instance, if you have a visible disability, you will want to disclose immediately and explain how the disability does not affect your ability to do the job or how you will accommodate for it.  You can also use your experiences with having a disability as part of the reasons why you are the best person for the job.  Always turn perceived negatives into positives.
